Gina Krizan, the LCO Behavioral Health Prevention Coordinator, needs your help by participating in the 2020 Community Prevention Assessment Survey. After submission of the survey, responders will get a link to leave contact information for incentives for filling out the survey which include $10 gas vouchers, $10 Dairy Queen, $10 Koobie's, $10 Coops Pizza and $10 Hayward Cinema.
