There are some changes to the 2021Verification Form that everyone needs to be made aware of. First, you can no longer fax in your Verification Form because forms and valid IDs were illegible and prevented the Enrollment Department from verifying tribal membership .
Second, many forms have been submitted without indicating on the second page whether a tribal member will be picking up their payment in person or having it mailed. If you submit a form without checking one of these boxes on the second page, your payment will be mailed to you.
Please be sure to submit a clear copy of your tribal ID or other form of valid ID with your verification form. Also please make sure your verification form is complete and legible otherwise your payment may be delayed.
